Join Our Wait list

Thank you for your interest in Functional Medicine with Dr. Anita. Please complete the form below to be placed on our wait list. 

Once you are on our wait list, you will receive an email on the day our waitlist opens, so it is important to pay attention to your email. We will only be accepting a limited number of new patients, and it will be a first-come, first-served situation. Our next wait list opens AUGUST 1, 2025, for additional information you can call our office at 419-615-7157. The email will outline specific requirements to become a patient of uDemand Health, such as commitment, finances, and an understanding of who we are and what we do. After you have read through the email, give us a call and we can move forward with scheduling an appointment. Patients who do not get a chance to be scheduled will be placed back on our waitlist and will have another opportunity to try again.

Wait list enrollment

Until we are able to see you, here are a few recommendations:

  1. Use our referral network on our website for other helpful individuals we think are great local to our area who can assist in providing the best care. 
  2. Live a healthy lifestyle with a balanced diet and regular movement. This will physical health by managing weight, reducing the risk of chronic diseases, and boosting energy levels. It strengthens the heart, immune system, and muscles while enhancing digestion and sleep quality. Mentally, it helps reduce stress, anxiety, and depression, while improving mood, focus, and self-esteem. Over time, these habits increase longevity, promote a higher quality of life, and foster better social interactions, contributing to overall well-being and a more fulfilling life. 
  3. Keep your stress levels down as best as you can. Managing stress also promotes better sleep quality, allowing for restful nights and improved overall energy levels. Physically, it lowers the risk of heart disease, high blood pressure, and other health issues associated with chronic stress. A calmer mind boosts your immune system, enhances cognitive function, and increases focus and productivity. 
  4. We encourage you to continue getting your yearly labs and following up with any specialists you see and your primary care provider.
